("PSI") | Effective Date: July 21, 2026 | Last Updated: July 21, 20261. Program DescriptionBy opting in to receive text messages from Planned Systems International, Inc. ("PSI," "Company," "we," "us," or "our") through the PSI SMS Notification Program (the "Program"), you agree to receive recurring SMS/text messages related to credentialing and privileging process updates, document requests and submission status updates, status notifications and reminders, and system alerts and support ticket updates, at the mobile telephone number you provided.2. Consent to Receive MessagesYou may opt in to the Program through a direct and verifiable method, including a PSI system enrollment form, an electronic submission (e.g., SharePoint or Power Apps), or documented consent collected during onboarding. By opting in, you expressly consent to receive text messages from or on behalf of PSI at the mobile number provided, including messages sent using an automatic telephone dialing system or other automated technology, as permitted under the Telephone Consumer Protection Act (TCPA), 47 U.S.C. § 227, and its implementing regulations.Consent to receive Program messages is not a condition of receiving PSI’s credentialing, privileging, or support services. You may opt out at any time as described in Section 5 below.3. Message Frequency Message Frequency Disclosure Message frequency varies based on your interaction with PSI services and program activity. There is no fixed number of messages per period — you will generally receive a message only when there is an update relevant to your credentialing, privileging, document, or support request.4. Message and Data RatesMessage and data rates may apply. Message and data rates may be charged by your wireless carrier and depend on your specific mobile plan. PSI does not charge for text messages sent as part of the Program, but standard messaging and data rates from your carrier will apply to all messages sent and received. Contact your wireless carrier for details about your messaging and data plan.5. How to Opt Out (Cancel) Opt-Out Instructions You can cancel the Program at any time. Reply "STOP" to any message you receive from PSI. After you send "STOP," we will send you a reply message confirming that you have been unsubscribed.PSI SMS Terms & Conditions Page 2After this, you will no longer receive text messages from this Program. If you want to rejoin, re-enrollusing one of the opt-in methods described in Section 2.You may also opt out by replying with any of the following keywords: STOP, STOPALL, UNSUBSCRIBE,CANCEL, END, or QUIT. If you experience issues with the opt-out process, please contact us using theinformation in Section 15.6. Service Availability & Carrier LiabilityThis Program depends on your wireless carrier’s network and device compatibility. Message delivery is subject tomobile carrier capabilities, and PSI is not responsible for delayed or undelivered messages caused by yourcarrier, network conditions, or device issues.PSI reserves the right to change the telephone number, system, or process used to operate the Program at anytime, and will provide notice of material changes where required by law. PSI also reserves the right to modify, addto, or discontinue the Program, or any features of the Program, at any time.8. Types of Messages• Credentialing and privileging process updates• Document requests and submission status updates• Status notifications and reminders• System alerts and support ticket updates9. Use LimitationsSMS is not intended for emergency communications. Do not use the Program to report emergencies or urgentsafety issues — contact emergency services or your PSI point of contact directly by phone.PSI will not transmit sensitive personal or medical information via SMS. Where sensitive information is required,you will be directed to a secure system (e.g., secure email or a PSI portal).10. PrivacyPSI respects your privacy and is committed to protecting your information in connection with the Program.Information Collected: PSI collects limited information necessary to support SMS communications, includingyour mobile phone number, communication preferences, and message interaction data (e.g., opt-in and opt-outrequests).PSI SMS Terms & Conditions Page 3How Information Is Used: This information is used solely to deliver SMS notifications, manage your subscription preferences, and support operational communication needs.Data Sharing: PSI does not sell your information or use it for marketing. Information may be shared only as necessary to deliver SMS services through telecommunications providers or to meet legal or regulatory requirements.Data Security: PSI uses appropriate safeguards to protect your information. Sensitive personal or medical information is not sent via SMS.Your Choices: You may opt out at any time by replying STOP, or request assistance by replying HELP.11. EligibilityTo participate in the Program, you must be an active or prospective provider, applicant, or authorized point of contact engaged in PSI’s credentialing and privileging processes (or another PSI program that uses this Program), and you must enroll using one of the opt-in methods described in Section 2. By enrolling, you represent that the mobile number provided is yours or that you have permission to enroll it.12. Disclaimer of Warranties & Limitation of LiabilityThe Program is provided on an "as is" and "as available" basis. PSI makes no representations or warranties of any kind, express or implied, regarding the Program, including without limitation the timeliness, deliverability, or accuracy of any messages sent through the Program. To the fullest extent permitted by applicable law, PSI disclaims all liability for any delays or failures in message delivery caused by your wireless carrier, network outages, device incompatibility, or other circumstances outside PSI’s reasonable control.13. Changes to These TermsPSI may update or modify these Terms at any time. Changes will be posted on this page with an updated "Last Updated" date. Your continued use of the Program after any changes are posted constitutes your acceptance of the revised Terms.14. Governing LawThese Terms are governed by the laws of the Commonwealth of Virginia, without regard to its conflict of laws principles, and by applicable U.S. federal law, including the TCPA and CTIA Messaging Principles and Best Practices.15.
